Tutorial 8 - Navigate through "My Facility Forms" - Transcript

Under "facility forms in progress" you'll see the form that you recently created. Click on the blue drop down arrows on the right to see what is missing from the form. Items that are complete will have a green checkmark, items that are incomplete will have a yellow exclamation mark, and items that are optional will have a blue exclamation mark.

On the left of the page, you'll see a column that says "enter tombstone information". Tombstone information is your basic project information and project data. The column next to it, "update self assessment" is where you will evaluate risk statements for your application, which then determines your filing requirements. It doesn't matter what order you complete these in. You can start wherever you want, save your information, log out, and come back at a later time, if you wish.

Over on the far right, there are three important buttons. They are the "view requirements checklist",  "the submit button", which will only become active when you've completed your application, and the "delete project" button.

Click on one of the hyperlinks with an "incomplete" symbol. This is where you can start filling out the information needed for your application. Throughout the application, the blue drop down arrow near the top of the page will tell you which fields need to be entered before you can submit that section of the application.

When filling out the self asssessment portion of the form, make sure you read the statements clearly before answering. If you need aditional information, or do not understand the question, click on the "guidance" link next to the questions, which will help clarify any questions you have.

Notice that some of the statements ask you to pick one option from a list, and others ask you to agree or disagree with a statement. For the agreement statements, check the box if you answer "yes" or leave it uchecked in your answer is "no".

When you've finished answering the questions, make sure you click on "Yes, I have completed addressing all statements listed above" and remember to save the page. If you've answered everything that is required, it says "this section is ready to submit" at the top. Now, click the green arrow to see what filing requirements are needed for this section based on your answers. You may have no filing requirements, or one or more of your answers to statements in the Self Assessment may have triggered a requirement to submit information.  If you are required to attach a filing requirement, it will say "this section is not ready to submit'. In some cases, You can obtain a template to fill out. Once you have completed the information required, create a PDF copy of the information and upload your attachment. To continue, press the green arrow. After you press the green arrow, you will automatically be brought to the next section of the application form.

At any time, if you want to stop and continue your application later, or skip to a different section, use the home button to bring you back to the main page.
